DEFINITION
The Stewart Community Home is a Personal Care
Home which provides housing and assistance to elderly and disabled people from
the Chattahoochee Valley and surrounding area. By
definition, a Personal Care Home is a facility which provides housing, food
service, and personal services (monitoring of medication, activities of daily
living) for two or more adults.
The original Stewart Community Home was opened in
1976 by Sarah & Harold Stewart. In April
of 1994 we moved from two locations on 2nd and 3rd Avenues, to our present
facility at 1125 15th Street.
Formerly the Edwina Wood
Elementary School, we are
currently equipped to house 72 residents.
MISSION STATEMENT
The Stewart Community Home provides permanent or transitional safe housing for those homeless and disabled adults requiring limited personal care by providing nutritious meals, medical stabilization programs, and activities to maximize essential life skills.
VISION
To improve the quality of life for the homeless and disabled population of the Chattahoochee Valley, regardless of their ability to pay, and be viewed as a model agency providing ongoing services to this special needs population.
RESIDENT POPULATION
At present, about 80% of our resident population
consists of individuals with a mental health disability or an intellectual
impairment. Most of these residents
have some type of chronic mental disorder, ranging from schizophrenia to
depression, as well as, other diagnoses. We also have dual diagnosed individuals
(mental illness and substance abuse).
The remaining 20% of our residents have various forms of physical
disabilities.
These people are not impaired enough to require
hospitalization or to be institutionalized on a permanent basis, but they are
unable to live in the community without supervision and the services we offer.
Some of our residents have no family members.
Others have families who for one reason or another, cannot, or will not,
care for them at home.
